Incident Narrative

An employee drove a forklift with a bundle of wire (4500 lbs.) on the forks to the door of an intermodal container and parked it. The employee got off the forklift and went inside the intermodal container to adjust dunnage. As the employee was adjusting dunnage the bundle of wire stationed on the forks of the forklift fell and pinned the employee between a wire bundle inside the container and the wire bundle that fell from the forklift. The employee sustained a fractured sternum, a fractured rib, and a fractured right forearm.
