Incident Narrative

On 3/9/16 at 9:30am an employee had serviced the planer, removed his lock, and started the machine. He observed a piece of wood near the point of operation and reached towards the planer blades to remove the wood and his fingers contacted the rotating planer blades resulting in amputation of part of the right-hand index and middle fingers.
