Incident Narrative

On March 30, 2016, at approximately 11:00 a.m., an employee was working from a 12-foot fiberglass step ladder, preparing to relocate a 2-inch vent line from a receiver tank. Steam and hot water came out of the vent, causing the employee to lean back, lose balance, and fall to the concrete floor below. The employee suffered a right hip fracture, multiple right rib fractures, and a right forearm laceration, requiring hospitalization. An energy control procedure was not used at the time of the incident.
