Incident Narrative
An employee was stepping off an A-frame type ladder. As he placed his weight on his back foot, he slipped and lost his balance. He fell backward and landed on top of an elevated welded pipe gas line, breaking three ribs and suffering a slightly collapsed lung on his right side.
