Incident Narrative
An employee was trying to cage a brake chamber with a caging bolt. The caging bolt was too short, so the employee applied shop air to the chamber to connect the bolt. As the employee was installing a fitting on the chamber to connect it to the air hose coupling, the employee's left middle finger was caught and crushed between the chamber and bolt washer.
