Incident Narrative
An employee was changing the air line on a three-station pneumatic press. Her finger was in a porthole on the equipment when she connected the air line to the machine. The air caused a piston in the machine to be actuated while her finger was in the hole, amputating her right pinky finger between the first and second knuckle.
