Incident Narrative

An employee was attempting to reach and adjust a chain that was holding up one of the very large bags in the baghouse. The employee crawled on top of the bags, which are held in place by angle irons 11 inches apart. While crawling toward the bag, the employee lost his grip and fell through the 11-inch gap. He fell 37 feet to the ground, breaking a foot and fracturing a vertebra.
