Incident Narrative

An employee was moving storage racks with a forklift when the forklift became stuck in the mud. The employee was putting boards under the tires with help from a coworker. The storage rack fell from the forklift and struck the employee. He sustained a crushed pelvis, broken ankle, broken ribs, and a head injury.
