Incident Narrative

An employee was putting away a cart of headlight extensions in the store. The employee turned to exit the store and walked out into a moving forklift. The employee's right foot was injured. The second, fourth and fifth digits (toes) were fractured and the big toe was dislocated. The employee was hospitalized.
