Incident Narrative
An employee was putting wire on a U-shaped, steel channel frame, balanced on forklift tines, in order to attach the frame to an overhead crane. The steel frame began to fall backwards, and the employee tried to maintain its balance causing the employee's right pinky finger to get caught between the forklift frame and the fork tines. The employee's right pinky finger was removed from above the second knuckle.
