Incident Narrative
On February 29, 2016, at approximately 3:05pm, an employee suffered injuries after falling to the floor. The employee was walking out of the Director's office, caught a shoe on a chair, and fell to the tile floor resulting in a loss of consciousness, nasal fracture, laceration to the lip, and bruising.
