Incident Narrative

On 02/29/2016 an employee was grinding a steel plate when the work stock got wedged. The work stock flipped up pinching his thumb tip between the work stock and the grinding disc. The employee was taken to the hospital for medical treatment. The employee's right thumb tip was later amputated.
