Incident Narrative

An employee was transferring formic acid from a leaking tote to a new tote. The new tote's valve was open, which allowed the formic acid to run out onto the ground. The employee walked toward the tote, slipped, and fell onto the formic acid, suffering third degree burns (right leg, backside, right ear, and hands) that required hospitalization.
