Incident Narrative

An employee standing on an 8-foot ladder was using a hammer to remove wooden blocks wedged between the door and the wall. When the last block was knocked out, the door shifted and caused the ladder to fall. The injured employee was approximately 5 feet high when his foot slipped off the rung and went through the ladder. He fell to the ground and fractured his lower leg.
