Incident Narrative
A maintenance employee was repairing a tapping machine. He was working underneath the machine. To access the conveyor belt, he was moving a product tray out of the way by pushing it. The tray came off the guides and tilted, and his finger was caught between the tray and the machine, amputating his left middle fingertip.
