Incident Narrative
An employee was cleaning out trash from the bed of a truck at the PSO yard. He stepped back and turned to throw out some trash and stepped on a plastic bottle. As he slipped and fell, his foot was caught between a fiberglass pole and the bed of the truck, breaking his leg.
