Incident Narrative

An employee was working on top of a steel girder when he stepped down to a concrete bend that was approximately 30 inches beneath the girder and diagram. While climbing down, the employee's retractable locked up, causing the employee to lose his balance, slip, and fall against the girder and diagram, fracturing his left leg.
