On Monday, February 8, 2016, at approximately 12:15 PM, an employee was operating a double rider (powered industrial truck). She pressed the wrong button and moved the truck forward instead of backwards, and she was pinned between the truck and a rack upright. She fractured her pelvis and was hospitalized.
