An employee was loading his trailer when he heard a hissing sound. He climbed the ladder to check the dome of the trailer, tightened the bolts and began to climb down the ladder. As he attempted to put his foot down on the second rung, he lost his footing and fell to the concrete below, fracturing his skull and suffering a concussion.
