Incident Narrative

An employee was struck in the head while removing a concrete form used to manufacture burial vaults. The form was being removed when a bar swung and hit the employee in the head, knocking him to the ground. The employee was standing on the floor at the time and was wearing a protective helmet. He was hospitalized after being knocked unconscious with a cut to the head.
