Incident Narrative

An employee was standing outside on a wooden walkway next to an entrance door. It had been raining and the wooden walkway was wet. The employee opened the door for a student. The student walked into the building. The door was about to slam shut so the employee twisted to grab the door and slipped on the wet walkway falling to the ground and breaking his tibia.
