Incident Narrative

An employee was pulling a cart (with caster wheels) that was carrying a 500-pound magnet. The caster wheel became caught on the concrete expansion gap, causing the cart and magnet to tip and fall on the employee. His trachea was torn in half, the internal walls of his chest were injured, and his scalp was lacerated. He was hospitalized.
