Incident Narrative

An employee was maneuvering a fifth wheel plate onto the bucket of a front-end loader. The employee and a coworker lost control of the plate, which fell flat. As it fell, the employee's right middle finger got pinned between the top of the front-end loader bucket and the fifth wheel plate, amputating the tip.
