On January 6, 2016, an employee was operating a cold header machine when a mobile welding cart became entangled with the coil wire being fed into the machine. The employee accidentally grabbed the wire, which began to advance in speed, and the employee's hand was pulled into the draw box. The employee's right ring finger was amputated.
