Incident Narrative
An employee was coming down an 8-foot extension ladder when he misplaced his descending foot and it entered the space between the rungs. As his leg went through the empty space, it came into contact with the ground, twisting him around and down the ladder. He dislocated his knee joint and broke his tibia.
