Incident Narrative
Employees were installing a smoke filtration system. The pieces were stored outside and became covered with snow. Employees used a forklift to bring the pieces inside the building. They tipped the forks on the truck downward to clear the snow off of the pieces to bring in. The pipe extension came loose and fell onto an employee's head causing a subdural bleed in the brain.
