Incident Narrative
The "perforator operator" employee was working behind the "P1" machine that applies plastic film wrap onto food product containers. The employee lifted the shield to make an adjustment while the machine was running and caught her left hand between two rollers. A portion of the ring finger on her left hand was amputated. No interlocks are used on this portion of the machine.
