Incident Narrative

The employee was working with another employee to set up a new operator valve system. While observing the operation of the valve system, one of the employees placed his hand into the opening of the plates and the plates closed onto his fingers. The employee's index and middle fingers were amputated. There was no guard on the machine at the time.
