Incident Narrative
An employee was standing on the back of a highway van trailer, pulling on a pallet with a hook. The hook disconnected from the pallet, causing the employee to fall off the trailer and hit the ground. He was hospitalized with broken ribs, a punctured lung, and a displaced ribcage.
