Incident Narrative

An employee was reloading pallets onto a truck and was returning the powered pallet jack to the truck. The employee walked the jack back onto the trailer in the space available on the lift gate. The jack moved to the right, running over the employee's foot and pinning his leg. He lost his balance and fell approximately 3 to 4 feet off the lift gate platform, twisting his right ankle and breaking his right leg below the knee.
