Incident Narrative

An employee was calibrating a machine. As he turned around, he lost his balance and fell. He tried to catch himself by grabbing the machine, which had an exposed mixture arm on it. The mixture arm was turning and caught the fingers on his right hand, amputating his index and ring fingertips.
