Incident Narrative
On 11/23/15, at about 2:30 a.m., an employee was operating a forklift to empty a hopper of garbage into a dumpster. He placed his hand on top of the fork while pushing up on the hopper tilt release lever. When the hopper tilted forward, the employee's finger was pinched between the top of the fork and the bottom of the dumpster. His left index fingertip was surgically amputated.
