Incident Narrative
A maintenance mechanic was using a powered hand drill with a 4 foot long ready rod (attachment) to clean out a pipe that was clogged with powder. When he inserted the rod into the pipe, the rod caught his gloved left hand lacerating his fingers. Because of personal medical reasons, the employee opted to have the tip of his index finger surgically amputated.
