Incident Narrative
An employee was repairing the overhead bracket, which holds the overhead guard, on a customer-owned forklift. The overhead guard fell and caught the employee's finger between it and the frame of the forklift. The employee's middle left-hand finger was amputated approximately 1/4" to 1/2" from the tip (middle of the fingernail).
