Incident Narrative
On November 15, 2015, at approximately 7:30 a.m., an employee was helping a coworker remove a 6.5-feet tall, 2.5-feet wide, 1.5-feet deep electrical panel. The panel was strapped to a two-wheel hand truck/dolly. The 450-pound cabinet became unstable and fell on top of him, pinning him beneath it. The dolly stayed strapped to the cabinet, and the cross bar on the dolly made contact with the employee's lower leg, fracturing it.
