Incident Narrative
Two employees were working on a labeler. The first employee was cleaning the fingers of the labeler, while the second was working on a sensor. When the second employee finished his work on the sensor, he closed a door on the machine, causing a rack to move forward and catch the first employee's left-hand middle finger. The first employee suffered an avulsion that required surgical amputation of the fingertip.
