Incident Narrative
A die setter was using an overhead crane to lift a die (approx. 2000-4000 lb.) so that he could put wood blocks underneath it . When the employee lifted the die, the load block was not centered under the die and caused the die to swing hitting the employee in the chest. The employee suffered a fractured sternum, 2 broken ribs, and a punctured lung.
