Incident Narrative

The injured employee was having a difficult time closing the latch of a shipping container door. This employee was standing beside the shipping container door by the handle while another employee, who was operating a forklift, pushed the latch of the container's door per the injured employee's command. The fork struck the injured employee's right hand middle finger resulting in a fingertip amputation.
