Incident Narrative

An employee working on a vertical polisher machine was installing a saw blade. He was hoisting a blade onto an arbor with a spacer. While aligning the blade to the bushing and arbor, he pushed the blade and bushing with one hand and operated the hoist with the other. The blade slipped off the hoist and fell on his left thumb, amputating the tip.
