Incident Narrative

An employee was operating a tie down machine in shock packaging. The employee was adjusting a tie strap while the tie down machine was cycling. The employee's finger was caught between the pusher head of the pneumatic tie down machine and the splatter washer resulting in amputation of the left index fingertip.
