Incident Narrative

The injured employee was cleaning at the end of the shift. She proceeded to put paper in a recycling bin and turned around to return to the Mega-cutter. As she passed by the weight scale she stepped diagonally across the ramp and caught the back portion of her foot on the ramp causing her to trip. The employee fell forward onto the floor landing on her right hip and fracturing it.
