An employee was checking for gasket pieces in the production line with a metal detector. The machine did not have LOTO and the safety guard had been removed at the time of the incident. The employee's fingers got caught in the machine. The third finger on the right hand was amputated at the middle of the distal phalanx and the fourth finger was avulsed at the distal tip.
