On October 12, 2015, at approximately 1:30am, an employee suffered the amputation of his left arm (up to the shoulder) after it became caught in the bridle roller, a power unit that moves the strip of metal at 300 feet per minute, while the employee was troubleshooting it. A guard was removed and the line lock procedure was not used.
