On Saturday 10/10/15 at approximately 5:00 AM, an employee's hand slid underneath a fixed guard at the Department 5540 Pleater. The employee's left index finger made contact with a belt and was pulled into a roller resulting in injury. Medical provider's diagnosis is that of a partial amputation of the left index finger.
