Incident Narrative

An employee had just finished picking up a donation and at 1260 Waterville Monclova Road in Waterville, Ohio. As the employee was closing the back sliding door of the box truck they were driving, the door came down on the top of the employee's right foot/toe. The employee lost balance, fell off the back of the box truck (approximately 3 to 4 feet), and struck their head against the concrete ground. The employee suffered a laceration to the right big toe and a possible skull fracture.
