Incident Narrative

An employee was putting a combo inside the trash compactor. Once the combo was inside, he turned on the trash compactor, but the equipment failed to operate normally. He attempted to adjust the combo, and as soon as the combo was adjusted, the compactor reacted quickly and crushed/amputated his right middle fingertip.
