Incident Narrative

An employee climbed a ladder and attempted to adjust the blow pin assembly of a blow mold machine. The machine was operating at the time. While the employee tried to move a bracket and tighten screws, the machine cycled and the assembly moved up, amputating the employee's right index finger below the first knuckle.
