Incident Narrative

On Thursday October 1, 2015, at the Suncast Distribution Center in Montgomery, Illinois, an employee was bent over working. Coworkers nearby were manually breaking down double-stacked pallets with boxed products. The last box (weighing 208 pounds) on the top of a double-stacked pallet shifted and fell onto the employee from overhead. The employee suffered a closed fracture to the pelvis, femoral vein damage, a pelvic hematoma, and a lower body injury.
