Incident Narrative

On or around 9/29/15, while taking measurements for installation of a lightening protection system, a worker on the second floor of Building 148 fell approximately 10 feet through a ladder access hole and fractured their skull. At the time of the incident, a guardrail was in place on 3 sides of the access hole.
