Incident Narrative

An employee was inside the block plant maintenance shop waiting for a coworker to finish cutting pieces of angle iron with an electric-powered reciprocating saw. While the saw was operating, the employee's finger contacted the belt running between the saw's electric motor and the flywheel. The employee's left index fingertip was caught between the belt and pulley, resulting in a partial fingertip amputation.
